  • The controls do not identify an owner or practical implementation step.
  • The assessment ignores vulnerable groups, interfaces, abnormal work or change.
  • The reviewer has not observed the work or consulted people with relevant knowledge where needed.

When to obtain professional or specialist help

Seek appropriate help when the organisation lacks competence for the hazards or legal questions involved, the potential consequence is serious, residual risk remains uncertain, specialist measurement or design is needed, or repeated issues suggest a wider management-system problem.
